Job summary

Elara Caring is seeking a Hospice Nurse Practitioner to join our team, delivering compassionate home-based hospice and palliative care.

You will perform routine and emergency visits, coordinate with physicians and the care team, educate staff, manage the Plan of Care, and support patients and families with dignity and respect.

This role includes documenting findings, maintaining CMS-compliant records, and contributing to clinical guidelines to raise the standard of care.

Qualifications

• Current NP license in the state of practice.
• Master’s degree in nursing and national NP certification.
• 1+ year of clinical nursing experience.
• Active DEA certification if state allows prescription of controlled substances by APPs.
• Ability to lift up to 50 pounds and physically assist patients and equipment.
• Valid driver’s license or reliable transportation to travel between patient homes.
• 1+ year post-graduate hospice/palliative care experience preferred.
• Experience in Family Medicine, Geriatrics or Psychiatry preferred.

Responsibilities

• Complete routine and emergency face-to-face recertification visits for hospice patients.
• Assess patient needs, document findings, and coordinate with physicians and the care team.
• Provide bedside palliative care, education, and support to patients and families.
• Triage patient needs to prevent avoidable hospitalizations.
• Maintain CMS-compliant clinical records, billing, and coding.
• Collaborate with Medical Director and interdisciplinary team to coordinate care.
• Provide training and continuing education to clinical staff.
• Assist in developing clinical practice guidelines and standards.
