Job summary

Elara Caring is seeking a Hospice Certified Nursing Assistant (CNA) to join our home health team. The role supports patients in their home setting, following a Plan of Care under RN supervision and collaborating with care teams to ensure safety and quality of care.

The ideal candidate has a high school diploma or GED, at least 1 year of hospice or nursing assistant experience, and a valid driver’s license. Travel up to 50% is required.

Qualifications

• High School Diploma or GED equivalent required.
• 1 year of Hospice Aide or Nursing Assistant experience required.
• State-established Hospice Aide training program meeting 42 CFR §484.36 requirements.
• Home health experience preferred.
• Travel up to 50% of the time.
• Valid driver’s license and reliable transportation.

Responsibilities

• Verifies Plan of Care and provides care per RN delegation and physician orders.
• Documents care and updates patient profiles.
• Communicates changes to RN Case Manager and physicians.
• Initiates patient safety actions and reports unsafe conditions.
• Assists patients with self-care and daily activities.
• Collaborates with RN Case Manager during business hours.
• Supports as needed under RN or physician supervision.
