Job summary

Elara Caring is seeking a Hospice Nurse Practitioner to deliver compassionate, patient-centered care in the home setting. You will perform routine and crisis visits, assess symptoms, document findings, and collaborate with physicians to manage the Plan of Care.

You will provide bedside palliative care, educate families, and help triage needs to prevent unnecessary hospitalizations. Strong documentation, CMS compliance, and team teaching are essential.

Qualifications

• Current NP license in the practicing state.
• Master’s in Nursing with NP certification.
• 1+ year clinical nursing experience.
• DEA certification, if allowed.
• Ability to lift up to 50 pounds and travel between homes.

Responsibilities

• Complete routine and emergency face-to-face visits for hospice patients.
• Assess needs, document findings, and collaborate with physicians and team.
• Provide bedside palliative care and education to patients and families.
• Triage needs to prevent avoidable hospitalizations at home.
• Maintain records, billing, and coding per CMS guidelines.
• Coordinate with Medical Director and team to ensure quality care.
• Provide training and continuing education to staff.
• Help develop clinical guidelines and standards.
