Job summary

Elara Caring seeks a Hospice Nurse Practitioner to deliver in-home hospice and palliative care. You will conduct routine and emergency visits, assess patient needs, document clinical findings, and collaborate with physicians to manage symptoms and the Plan of Care.

The role requires NP licensure, a master’s degree in nursing, and NP certification, with 1+ year of clinical nursing experience. DEA certification may be required where permitted. A driving role involves visiting patient homes.

Qualifications

• Current license as a Nurse Practitioner in the state of practice.
• Master’s degree in nursing and national NP certification.
• 1+ years of clinical nursing experience; hospice/palliative care preferred.
• Active DEA certification if applicable by state law.

Responsibilities

• Perform routine and emergency face-to-face recertifications for hospice patients.
• Assess patient needs, document findings, and collaborate with physicians.
• Provide bedside palliative care and education to patients and families.
• Triage patient needs to prevent unnecessary hospitalizations.
• Maintain accurate patient records and billing documentation.
• Collaborate with Medical Director and care team to coordinate care.
• Provide training to clinical staff on hospice and palliative care.
• Develop and promote clinical practice guidelines and standards.
