Job summary

Elara Caring seeks a Hospice Nurse Practitioner to lead home-based palliative care visits in Topeka, KS. You will perform admissions, recertifications, and symptom management for hospice patients, collaborating with physicians and the care team to advance the Plan of Care.

The role requires NP licensure, a Master’s in nursing, and 1+ year of clinical nursing experience, with DEA certification if allowed. A valid driver’s license and reliable transportation are essential.

Qualifications

• Requires current NP licensure in state of practice.
• Must hold Master’s degree in nursing and NP certification.
• 1+ years of clinical nursing experience.
• Maintain active DEA certification if allowed.
• Ability to lift/move patients and equipment up to 50 pounds.

Responsibilities

• Complete routine and emergency face-to-face recertification visits for hospice patients.
• Assess patient needs and document findings; collaborate with physicians and care team.
• Provide bedside palliative care, education, and family support.
• Triage patient needs to prevent avoidable hospitalizations by home care.
• Maintain patient records, billing, and CMS-compliant documentation.
• Coordinate with Medical Director and interdisciplinary team to ensure quality care.
• Provide training/education to clinical staff on hospice/palliative care.
