Job summary

Elara Caring in Oklahoma City is seeking a Hospice Nurse Practitioner to deliver compassionate in-home hospice care and lead symptom management for patients. You will conduct routine and urgent visits, assess needs, and collaborate with physicians to ensure high-quality care.

The ideal candidate holds an Oklahoma NP license, a master's in nursing, and 1+ year hospice experience, with ability to lift up to 50 pounds and travel between homes. A valid driver's license is required.

Qualifications

• Current NP licensure in Oklahoma or state of practice.
• Master’s degree in nursing and national NP certification.
• 1+ year clinical nursing experience.
• Maintain DEA certification if allowed.
• Ability to lift up to 50 pounds and travel between patient homes.

Responsibilities

• Complete routine and emergency face-to-face recertification visits for hospice patients (admissions, transfers, extended stays).
• Assess patient needs, document findings, and collaborate with physicians to manage symptoms.
• Provide bedside palliative care, education, and support to patients and families.
• Triage patient needs to prevent avoidable hospitalizations by addressing care needs at home.
• Maintain accurate patient records, billing, and CMS-compliant documentation.
• Collaborate with Medical Director and interdisciplinary team to coordinate care.
• Provide training to clinical staff and help develop clinical guidelines.
