Ideal candidates hold a NJ Advanced Practice Nurse license with prescriptive authority (CDS/DEA), a national Adult or Geriatric NP certification, and 2+ years of APN or RN experience.

Qualifications

• Master’s degree in Nursing from an NLN accredited program.
• NJ Advanced Practice Nurse license with prescriptive authority; maintain CDS and DEA.
• National certification as an Adult or Geriatric Nurse Practitioner.
• 2+ years APN experience and/or significant RN experience.
• Excellent communication, organizational and interpersonal skills.
• Strong computer skills.

Responsibilities

• Assesses, diagnoses and develops the plan of care for homebound patients.
• Promotes high quality care and life for patients and families.
• Monitors patient condition and response to treatment.
• Acts as agency representative with patients, families and other health entities.
• Participates in QAPI program and agency in-services.
• Adheres to infection control and safety policies.
• Adheres to absenteeism and appearance policies.
