Qualifications
- Licensed as a NP in the state of practice
- Federal DEA license or eligibility
- Geriatric training or experience preferred
- Active CPR/BLS certification
- Board certification as a healthcare professional

Responsibilities
- Provide home-based primary care to high-acuity patients, focusing on patient needs rather than volume
- Deliver patient-centered care across home health, geriatrics, and hospice settings
- Plan and coordinate care with an interdisciplinary team
- Ensure comprehensive, quality care with appropriate documentation and follow-up
