Job summary

UnitedHealthcare is seeking an ECF CHOICES Support Coordinator to manage a panel of members with developmental disabilities, focusing on medical, behavioral, and socioeconomic needs to optimize care utilization and quality. This home-based role includes approximately 75% field responsibilities.

Candidates should reside within a 50-mile radius of Williamson County, TN, and be prepared to travel up to 75% within advertised counties.

Qualifications

• 4-year or higher degree in social work, nursing, education, rehabilitation counseling, or related human service field
• RN or LPN licensure required
• Meet federal requirements for QDDP or QIDP
• 5+ years as independent Support Coordinator or HCBS waiver case manager

Responsibilities

• Engage members to complete comprehensive needs assessments (medical, behavioral, functional, cultural, socioeconomic)
• Develop and implement person-centered care plans for health, wellness and SDoH
• Coordinate with internal care teams, providers, and community resources
• Provide referrals and linkages to housing, pharmacy, peer support, and other services
• Support plan execution and monitoring to improve care quality
