Job summary

UnitedHealthcare, part of UnitedHealth Group, seeks an ECF CHOICES Support Coordinator to oversee a panel of members with intellectual or developmental disabilities. This home-based role involves ~75% field work and travel within Tennessee counties to meet members where they are.

You will conduct comprehensive assessments, develop individualized care plans, coordinate with providers, and link to housing, transportation, and other supports to improve outcomes.

Qualifications

• Degree or professional licensing as listed in requirements.
• Valid driver's license and ability to travel up to 75%.
• Experience with 1915(c) HCBS Waiver and QDDP/QIDP trainings is required.

Responsibilities

• Complete comprehensive needs assessments with medical, behavioral and social needs.
• Develop and implement person-centered care plans for chronic conditions and wellbeing.
• Collaborate with care team and community resources to execute plans.
• Provide referrals and linkages to internal and community-based services.
