Job summary

RHA Health Services is seeking compassionate Family Model Providers in Missouri to support adults and children with I/DD at home. As an independent contractor, you offer a safe, comfortable living environment and work under a multidisciplinary team.

Responsibilities include understanding the person’s history, assisting with meals, self-care, transportation, medical appointments, and community participation, while honoring choices and privacy. Training and supervision are provided.

Qualifications

• HS Diploma or GED is required.
• Age 18 or older with valid driver's license and reliable transportation.
• Ability to pass criminal background checks, including OIG exclusions, and drug screening.
• Proof of home ownership or a renter's lease and home insurance.

Responsibilities

• Support individuals in home, at work, and in the community based on their goals.
• Help with meals, self-care, rest, recreation, and transportation to activities.
• Respect choices and culture, protect privacy and dignity.
• Attend team meetings and participate in supervision as contracted.
