Job summary

RHA Health Services, LLC seeks compassionate individuals to be Family Model Providers, independent contractors offering in-home supports for people with I/DD. We train and support you to empower individuals in their own homes within a supportive, caring framework.

Responsibilities include understanding personal history, supporting meals and daily activities, ensuring privacy and dignity, and coordinating transportation and medical appointments while maintaining a safe home environment.

Qualifications

• High School Diploma or GED is required.
• 18 years old or older.
• Valid Driver’s License; reliable transportation with valid vehicle insurance.
• Ability to pass criminal background and registry checks and a drug screen.
• TB screening for all members of household.
• Will work as an Independent Contractor.
• Pre-employment screening: criminal background check, OIG exclusions, Child Abuse Registry, and other credentialing as required.

• Seek to understand the individual in the context of their personal history, social and family networks, and their hopes and dreams for the future.
• Support the individual in eating nutritious meals and snacks consistent with dietary needs, resting, daily self-care activities, leisure/recreation, education and working.
• Provide support according to the person’s abilities and interests while ensuring privacy and dignity.
• Prepare, teach and assist the person in food preparation, and cleaning of the home.
• Honor the choices, preferences and culture of the person you support.
• Assist in providing opportunities for social, emotional and spiritual needs to be met.
• Assist with transportation and scheduling of all medical, therapy, vocational and recreational activities.
• Ensure that the home environment is safe and in good condition.
• Implement services and supports to address goals identified through the Person‑Centred Plan (medical, psychiatric, recreational, spiritual, and emotional needs).
• Assist the person by attending treatment team meetings and facilitating communication related to individual needs, response to service and supports, and progress toward goals.
• Participate in supervision as agreed upon in the RHA supervision contract.

Family Model Provider (Independent Contractor)

Family Model Provider is an Independent Contractor. Independent Contractors are not employees and are not eligible for company benefits.

Bring a desire to serve others and share your home; RHA will provide the training and support to ensure your success.

• Prepare, teach and assist the person in food preparation, and cleaning of the home.
• Honor the choices, preferences and culture of the person you support.
• Assist in providing opportunities for social, emotional and spiritual needs to be met.
• Assist with transportation and scheduling of all medical, therapy, vocational and recreational activities.

Support Provided by RHA

• Support through a multidisciplinary team for crisis assistance and training (ProAct training).
• Assist in coordinating respite care.
• Provide supervision to help adjust to the new role; frequency determined collaboratively.

Requirements

• High School Diploma or GED.
• 18 years old or older.
• Documentation of home ownership: current mortgage statement or renter’s lease.
• Proof of homeowner’s or renter’s insurance or personal property insurance.
• Valid Driver’s License; reliable transportation with valid vehicle insurance.
• Ability to pass criminal background and registry checks and a drug screen.
• TB screening for all members of household.
• Will work as an Independent Contractor.
• Pre‑employment screening: criminal background check (including OIG exclusions, Child Abuse Registry, and Offenders Against Individuals with Developmental Disabilities), drug testing, education verification, and other credentialing as required.
