Family Model Provider (Independent Contractor)

We are hiring for a Family Model Provider (Independent Contractor) to support individuals with intellectual, developmental, or mental health and substance use challenges in a home setting. As a professional caregiver, you will help clients reach personal goals and live the life they choose.

Responsibilities

• Seek to understand the individual in the context of their personal history, social and family networks, and hopes and dreams for the future.
• Support the individual in eating nutritious meals and snacks consistent with dietary needs, resting (by providing sleeping accommodations), daily self‑care activities, leisure/recreation, education and work.
• Provide supports according to the person’s abilities and interests while ensuring privacy and dignity.
• Prepare, teach and assist the person in food preparation and cleaning of the home.
• Honor the choices, preferences, and culture of the person you support.
• Assist in meeting social, emotional, and spiritual needs.
• Assist with transportation and scheduling of all medical, therapy, vocational, and recreational activities.
• Ensure that the home environment is safe and in good condition.
• Implement services and supports to address goals identified through the Person‑Centered Plan, including medical, psychiatric, recreational, spiritual, and emotional needs.
• Assist the person by attending treatment team meetings and facilitating communication related to individual needs, response to service and supports, and progress toward goals.
• Participate in supervision as agreed upon in the RHA supervision contract.

Position Requirements

• High School Diploma or GED.
• 18 years of age or older.
• Documentation of home ownership or tenancy (current mortgage statement or renter’s lease).
• Proof of homeowner’s or renter’s insurance or personal property insurance.
• Valid driver’s license; reliable transportation with valid vehicle insurance.
• Ability to pass criminal background and registry checks as well as a drug screen.
• TB screening for all members of the household.
• Work as an Independent Contractor.
• Pre‑employment screening: complete criminal background check, drug testing, education verification, and other credentialing based on position requirements.
