Job summary

RHA Health Services, LLC is seeking AFL Professional Caregivers as Independent Contractors to support adults and children with I/DD in a home setting. Training includes ProAct and supervision from a multidisciplinary team to ensure successful integration into the care plan.

You will work in a home-based environment, offering nutrition support, daily living activities, and coordination of transportation and therapies, while respecting privacy and dignity.

Qualifications

• HS Diploma or GED is required.
• Applicant must be 18 years or older.
• Must be able to pass criminal background check and Drug Screen, plus Healthcare Registry.
• Ability to work as an Independent Contractor as part of AFL program.

Responsibilities

• Understand the individual’s history, family networks, and goals.
• Support the individual with meals, self-care, and daily activities at home.
• Respect privacy and dignity while providing supports.
• Assist with food preparation and home cleaning.
• Honor the person’s choices and culture; support social and emotional needs.
• Coordinate transportation and scheduling for medical and recreational activities.
• Ensure a safe home environment and address goals through Person Centered Plan.
