Job summary

RHA Health Services, LLC is seeking a Family Model Provider to welcome adults or children with intellectual and developmental disabilities into a home-based setting. As an Independent Contractor, you’ll provide daily living support, meals, transportation, and community access, while maintaining privacy and dignity.

Requirements include HS Diploma, 18+, valid driver’s license, home documentation, and background checks.

Qualifications

• Must be 18 years or older.
• Independent Contractor status with no company benefits.
• Provide living support in a home setting.
• Ability to pass criminal background and registries checks.
• Driver’s license and reliable transportation.

Responsibilities

• Understand the individual's history, networks, and goals.
• Support with meals, self-care, recreation, education, and work.
• Respect privacy and dignity while offering supports.
• Assist with food preparation and home cleaning.
• Honor the person’s choices and culture.
• Coordinate transportation for medical, therapy, and activities.
• Ensure a safe home environment and progress on goals.
