Job summary

RHA Health Services, LLC is seeking compassionate Family Model Providers in Jackson, TN to deliver in-home support for adults and children with I/DD and mental health needs. This is an independent contractor role with training, including ProAct, and flexible scheduling within a home setting.

You’ll work with a multidisciplinary team, ensuring safety, dignity, and progress toward personal goals while coordinating transportation and community activities. Training and supervision are provided.

Qualifications

• HS Diploma or GED required.
• 18 years old or older.
• Documentation of home ownership or renter’s lease.
• Proof of homeowner’s or renter’s insurance or personal property insurance.
• Valid driver’s license with reliable transportation and vehicle insurance.
• Ability to pass criminal background checks and drug screening.
• TB screenings for all household members.

Responsibilities

• Support individuals with disabilities in home and community settings.
• Assist with eating, self-care, meals, and daily activities.
• Prepare and assist with food preparation and home cleaning.
• Honor choices, preferences, and cultural backgrounds.
• Provide transportation coordination for medical, therapy and recreational activities.
• Ensure a safe home environment and participate in treatment team communications.
• Attend supervision as agreed and support caregiver needs.
