Family Model Provider (Independent Contractor)

Family Model Provider is an Independent Contractor. Independent Contractors are not employees and are not eligible for company benefits.

We are looking for caring individuals who share our dedication to providing a supportive environment for persons with intellectual and/or developmental disabilities (I/DD) in their home. Bring a desire to serve others and share your home; RHA will provide the training and support to ensure your success.

Responsibilities

• Seek to understand the individual in the context of their personal history, social and family networks, and their hopes and dreams for the future.
• Support the individual in eating nutritious meals and snacks consistent with dietary needs, resting (by providing sleeping accommodations), daily self‑care activities, leisure/recreation, education and working.
• Provide support according to the person’s abilities and interests while ensuring privacy and dignity.
• Prepare, teach and assist the person in food preparation, and cleaning of the home.
• Honor the choices, preferences and culture of the person you support.
• Assist in providing opportunities for social, emotional and spiritual needs to be met.
• Assist with transportation and scheduling of all medical, therapy, vocational and recreational activities.
• Ensure that the home environment is safe and in good condition.
• Implement services and supports to address goals identified through the Person‑Centred Plan (medical, psychiatric, recreational, spiritual, and emotional needs).
• Assist the person by attending treatment team meetings and facilitating communication related to individual needs, response to service and supports, and progress toward goals.
• Participate in supervision as agreed upon in the RHA supervision contract.

Support Provided by RHA

• Support through a multidisciplinary team for crisis assistance and training (ProAct training).
• Assist in coordinating respite care.
• Provide supervision to help adjust to the new role; frequency determined collaboratively.

Requirements

• High School Diploma or GED.
• 18 years old or older.
• Documentation of home ownership: current mortgage statement or renter’s lease.
• Proof of homeowner’s or renter’s insurance or personal property insurance.
• Valid Driver’s License; reliable transportation with valid vehicle insurance.
• Ability to pass criminal background and registry checks and a drug screen.
• TB screening for all members of household.
• Will work as an Independent Contractor.
• Pre‑employment screening: criminal background check (including OIG exclusions, Child Abuse Registry, and Offenders Against Individuals with Developmental Disabilities), drug testing, education verification, and other credentialing as required.
