Job summary

Health Connect America, Inc. is seeking a Family Development Specialist to support clients in homes and communities. You will develop and implement treatment plans, provide individual, group, and family interventions, and coordinate care with the treatment team and supervisor.

The role emphasizes compliance, documentation, and active referral recruitment. Applicants should have a bachelor’s degree in a human services field and at least two years of relevant experience.

Qualifications

• Bachelor’s degree in a human services discipline is required.
• Minimum of two years experience working with children and/or families.

Responsibilities

• Develop individualized treatment plans and coordinate care with input from the treatment team and Clinical Supervisor.
• Provide individual, group, and family therapy interventions and skill-building.
• Attend and actively participate in weekly Treatment Team meetings.
• Link clients with services and resources identified in the treatment plan.
• Serve as liaison with professionals and agencies to meet client needs.
• Maintain accurate documentation of client interactions and progress notes.
• Actively recruit new referrals to maintain a full caseload.
• Provide 24/7 on-call support as required by the program.
• Ensure compliance with state regulatory bodies and COA standards.
• Participate in ongoing training to stay current with best practices.
