Job summary

Health Connect America, Inc. seeks a Family Development Specialist to support clients and families by creating and implementing treatment plans, delivering therapeutic interventions, and connecting them with needed services, working in community settings and client homes.

You will collaborate with a treatment team, maintain documentation, and recruit referrals while ensuring regulatory compliance and COA standards. This is a full‑time role with comprehensive benefits.

Qualifications

• Bachelor’s degree in a human services field is required.
• Minimum of two years experience working with children and/or families.

Responsibilities

• Develop individualized treatment plans and coordinate care for clients and families with input from the treatment team.
• Provide individual, group, and family therapeutic interventions.
• Attend and actively participate in weekly Treatment Team meetings.
• Link clients to services and resources identified in the treatment plan.
• Maintain accurate documentation of client interactions and progress notes.
• Actively recruit new referrals to maintain a full caseload.
• Provide 24/7 on-call support as required by the program.

Education
