Job summary

Lutheran Social Services of Wisconsin and Upper Michigan is seeking a compassionate CCS Case Manager to join our Comprehensive Community Services team in Dane County. This community-based role supports children, adolescents, adults, and families experiencing mental health challenges through a person-centered, recovery-focused approach.

Qualifications

• Master's degree in a human services-related field.
• Valid driver's license and reliable transportation.
• Ability to travel throughout Dane County to meet clients.
• Ability to work flexible hours, including occasional evenings based on client needs.

Responsibilities

• Coordinate services for CCS participants and develop individualized service plans.
• Provide psychoeducation and assist with treatment planning.
• Facilitate recovery-focused and person-centered team meetings.
• Connect clients to community resources and supports.
• Maintain accurate client records and documentation.
• Complete required reports and service documentation on time.
• Collaborate with treatment teams, schools, and other providers.
• Participate in supervision and staff development opportunities.
