Job summary

Benefis Health System is seeking a Home and Community-Based Case Management Nurse to deliver patient-focused care in home and community settings in Montana. You will join the multi-disciplinary Care Management Team to plan, assess, coordinate and monitor services for elderly or disabled beneficiaries to prevent hospitalization and maintain medical stability at home.

Requirements include RN or LPN licensure, nursing experience, and knowledge of case management.

Qualifications

• Graduate of an accredited school of nursing; RN preferred or LPN.
• Current state licensure as an RN or LPN.
• Certification in Case Management preferred.
• Three years of professional nursing experience.
• Knowledge of case management methods, practice and procedure.
• Knowledge of diagnostic and crisis intervention skills.
• Knowledge of long-term care needs and Medicare/Medicaid reimbursement.
• Budget management experience.

Responsibilities

• Responsible for the health service aspect of case management.
• Member of the Care Management Team coordinating planning, assessment and monitoring.
• Provide clinical assessment, medication review and management of chronic illnesses.
• Coordinate skilled services to prevent hospitalization and ensure medical stability at home.
• Follow licensing board requirements and Benefis Health System policies.
