Responsibilities

• Coordinate care management services for Montana Big Sky Waiver beneficiaries in home/community settings.
• Collaborate with a multidisciplinary Care Management Team to plan, assess, and monitor services.
• Support emotional well‑being and quality of life while meeting deadlines and handling changing situations.
