Commonwealth Care Alliance in Massachusetts is seeking a Care Delivery RN to collaborate with an interdisciplinary team, delivering primary care, in-home specialty care, and care coordination for a high-risk panel of patients with complex medical, behavioral, and social needs.

This role emphasizes care management, documentation, and active participation in care team meetings, with travel across assigned locations. Massachusetts RN licensure and BLS are required.

Competenze

• Associates degree in nursing required.
• Bachelor’s degree in nursing preferred.
• Five years RN experience in a high-touch clinical or home care setting required.
• Two years caring for patients with complex medical, behavioral health, and social needs required.
• Experience with electronic medical records strongly preferred.

Mansioni

• Deliver care and care coordination for complex patients within RN scope.
• Conduct basic health assessments and follow care plans.
• Escalate pertinent findings to APC within specified timeframes.
• Follow-up with patients by phone to ensure satisfaction.
• Document visits clearly and concisely in the EMR.
• Travel to member, provider, and community visits as needed.
