Job summary
CareSource/Commonwealth Care Alliance (CCA) seeks an Integrated Care Clinical Manager (RN) in Massachusetts to lead complex care management for dually eligible enrollees, coordinating medical, behavioral and social needs.
The role involves traveling to member homes and community sites, collaborating with providers, and guiding the care team to develop and implement holistic plans that improve outcomes and quality of life.
Qualifications
- Associates of Science in Nursing from an accredited program.
- Registered Nurse licensed in Massachusetts (active).
- Experience in care coordination or dual-eligible populations preferred.
Responsibilities
- Lead interdisciplinary care team to develop holistic care plans.
- Coordinate care across medical, behavioral and social services.
- Monitor enrollees and ensure timely follow-ups and transitions.
- Educate members and families about benefits and services.
- Travel to member homes and community settings as required.
