Qualifications

• MA RN license required.

• Bilingual language skills in English/Spanish, Portuguese or Vietnamese desirable.
• Cultural competency and teamwork in diverse settings.

Responsibilities

• Case management and care coordination of patients enrolled in C3.
• Deliver care coordination and follow-up, including referrals and transitions.
• Engage in care planning with medical, behavioral health and social services.
• Document care activities accurately in EHRs.
• Coordinate with hospitals, treatment programs and external partners.
