Job summary

Providence at Home with Compassus seeks a dedicated Occupational Therapist to deliver inpatient-level therapy in patients' homes. You will assess, plan, and implement personalized treatment and discharge strategies while collaborating with a multidisciplinary team to restore independence and quality of life.

The role requires an OT license, BLS certification, and ideally 1 year of post-acquired home care experience with adults. A flexible schedule and team support enhance the home-care mission.

Qualifications

• Current, unencumbered OT license in the state of employment.
• National Provider BLS – American Heart Association (Vendor Managed).
• Preferred 1 year of OT experience with adults in a home care setting.

Responsibilities

• Deliver comprehensive OT services in home settings, including assessment, treatment planning, implementation, and discharge planning.
• Evaluate clients’ physical, cognitive, psychological, and perceptual functions for home safety and independence.
• Develop individualized care plans with patients and families.
• Identify referrals affecting care considering medical, social, and emotional factors.
• Educate families on treatment plans, safety, and therapy goals.
• Adapt equipment and therapy techniques to home environments to promote safety.
• Independently manage caseload, schedule visits, and coordinate with the inter-disciplinary team.
• Supervise Home Health Aides and provide case management for COTAs.
