Providence at Home with Compassus seeks a Home-Based Acute Care Occupational Therapist to provide inpatient-level therapy in patients' homes, including evaluations, treatment planning, and thoughtful discharge planning.

You will work independently within a multidisciplinary team, manage a caseload, schedule visits, supervise Home Health Aides, and document promptly to meet agency guidelines. This role emphasizes compassionate care and restoring independence in the home.

Qualifications

• Required: Bachelor's degree in Occupational Therapy or Master's degree in Occupational Therapy.
• Preferred: 1 year of OT experience with adults in home health.
• State licensure as Occupational Therapist.

Responsibilities

• Deliver comprehensive therapy services including assessment, intervention, care planning, and discharge planning.
• Evaluate function in home settings to ensure safe independent living.
• Develop individualized care plans with patients and families.
• Supervise Home Health Aides and coordinate with the interdisciplinary team.
• Complete same-day documentation per guidelines.
