Job summary

The Wright Center is seeking a Prison Linkage Case Manager to support individuals living with HIV/AIDS transitioning from incarceration to community care. You will coordinate access to HIV treatment, social services, and transportation, while advocating for clients’ medical and social needs.

Collaborating with healthcare providers and correctional facilities, you will develop care plans, conduct assessments, and ensure compliant documentation, all in a culturally competent, patient-centered

Qualifications

• Bachelor’s degree in social work, psychology, sociology, or related field.
• Experience in case management, preferably HIV/AIDS services or related healthcare settings.
• Knowledge of HIV and re-entry processes; ability to advocate for clients.
• Basic Life Support (BLS) certification required.
• Excellent verbal and written communication skills.
• Demonstrated ability to maintain confidentiality and comply with program requirements.

Responsibilities

• Assess clients’ medical, psychosocial, and financial needs to understand individual requirements.
• Develop individualized care plans and service coordination plans with healthcare goals.
• Conduct intake assessments, reassessments, and maintain documentation in EMR.
• Perform post-release follow-up within two weeks of reentry to address barriers to care.
• Coordinate with healthcare providers to ensure access to HIV treatment and related services.
• Advocate for clients to receive appropriate medical care and social services; maintain client confidentiality.
