Job summary

The Wright Center for Community Health is seeking a Prison Linkage Case Manager in Scranton to support individuals living with HIV/AIDS within the criminal justice system. The role focuses on seamless transition from incarceration to the community and coordination of medical care, support services, and reentry planning.

You will collaborate with healthcare providers, correctional facilities, and community organizations; maintain confidential records; and advocate for access to antiretroviral

Qualifications

• Bachelor's degree in social work or related field.
• Knowledge of HIV preferred.
• Basic Life Support (BLS) certification required.
• Experience in case management, preferably HIV/AIDS services.
• Strong verbal and written communication skills.
• Understanding of criminal justice and re-entry processes.
• Ability to advocate for clients to access medical and social services.

Responsibilities

• Assess medical, psychosocial, and financial needs of clients.
• Develop individualized care plans based on assessments.
• Conduct post-release follow-up within two weeks of reentry.
• Coordinate access to medical care and antiretroviral treatment.
• Assist with navigating healthcare systems and resources.
• Maintain confidential client records and documentation.
