Job summary

The Wright Center for Community Health and Graduate Medical Education is seeking a RW Part B/C Medical Case Manager and Retention Coordinator. You will lead patient retention, coordinate care, and manage eligibility for Ryan White Part B and Part C programs within an HIV care program.

This role emphasizes outreach to re-engage patients, data-driven care coordination, and collaboration with interdisciplinary teams to improve care outcomes.

Qualifications

• Bachelor’s degree preferred; four years in a related field in lieu of degree
• Knowledge of HIV preferred
• BLS certification required
• Excellent verbal and written communication skills
• Demonstrated cultural competence with diverse populations

Responsibilities

• Conduct comprehensive assessments of clients’ medical, psychosocial, financial, and support service needs
• Complete intake assessments, reassessments, recertifications, and all case management documentation
• Develop and implement individualized service coordination plans
• Coordinate access to medical care and supportive services
• Support medication adherence and education on treatment importance
• Assist clients navigating healthcare systems, insurance, and community resources
• Advocate for clients to ensure access to care and services
• Maintain eligibility documentation in CAREWare and EMR
• Enter case notes within required timeframes and ensure accurate reporting
