Job summary

The Wright Center for Community Health and Graduate Medical Education seeks a Prison Linkage Case Manager to support individuals living with HIV/AIDS within the criminal justice system. Qualifications

• Bachelor's degree in social work, psychology, sociology, or related field.
• BLS certification required.
• Knowledge of HIV preferred.
• Experience in case management, preferably in HIV/AIDS services or related healthcare settings.

Responsibilities

• Conduct assessments of clients' medical, psychosocial, and financial needs.
• Develop individualized care plans based on assessments and outline healthcare goals and strategies.
• Coordinate post-release follow-up within two weeks of reentry to assess needs.
• Collaborate with healthcare providers to facilitate access to HIV treatment and services.
• Maintain accurate client records in CAREWare and Electronic Medical Records.
• Participate in interdisciplinary team meetings and quality management initiatives.
• Educate clients about HIV prevention, treatment options, and available support services.
