Job summary

Metropolitan Charities, Inc. seeks a Medical Case Manager to provide client-centered services for individuals living with HIV/AIDS. You will link clients to healthcare, coordinate treatment, and ensure timely access to services in a community setting.

The role includes assessing needs, developing plans, coordinating resources, counseling on adherence, and documenting progress while advocating for access to public programs.

Qualifications

• Must speak, read, and write proficiently in English; bilingual (Spanish) preferred.
• Knowledge of HIV/AIDS and related care concepts.
• Cultural sensitivity to diverse backgrounds and psychosocial issues.
• Able to use word processing and basic computer software.

Responsibilities

• Conduct initial assessments of service needs and develop individualized plans.
• Coordinate services and follow-up on medical treatments and care plans.
• Monitor client progress and re-evaluate plans at least every 6 months.
• Provide treatment adherence counseling and collaborate with care teams.
• Document activities accurately in case notes and related forms.
• Advocate for clients to access Medicaid, Medicare and other programs.
• Assist with referrals for food, transportation, and emergency aid.
