Job summary

New North Citizens’ Council seeks a Temporary HIV/Aids, Health & Prevention Program Outreach Worker to deliver community-based outreach, prevention education, counseling, testing, and resource navigation to individuals affected by HIV/AIDS, hepatitis C, STIs, and substance use in Springfield, MA.

Reporting to the Program Supervisor, you will engage underserved populations, promote harm reduction, and connect participants with healthcare, treatment, and supportive services to improve individual

Qualifications

High school diploma or equivalent required.
Knowledge of HIV/AIDS, hepatitis C, STIs, substance use, and related public health issues.
Ability to engage diverse populations with culturally responsive approaches.
Strong interpersonal, communication, organizational, and documentation skills.
Willingness to complete required training and certifications.

Responsibilities

Conduct community-based outreach in neighborhoods and other locations to promote prevention and health services.
Engage underserved communities to increase access to education, testing, harm reduction, and services.
Provide prevention education, counseling, and testing per program protocols.
Coordinate referrals to healthcare, treatment, and support services.
Maintain confidential participant records and reporting.
Attend required trainings and collaborate with partners.
