Job summary

Qualifications

• High School Diploma or equivalent required.
• 0-1 years of relevant experience, required.
• Experience in HIV/STI prevention, community outreach, public health or related field strongly preferred.
• Ability to work effectively with diverse populations in a respectful, nonjudgmental manner.

Responsibilities

• Contacts patients; schedules appointments for consultations, agency meetings and/or diagnostic tests.
• Collects and organizes data required for statistical reports and completes reports.
• Attends and participates in meetings as appropriate.
• Sets-up files and maintains records.
• Provides incoming and in-process orientation.
• Based on position, may provide bilingual translation and/or guidance.
• Coordinate outreach activities to raise HIV, HCV, Syphilis, PrEP, PEP awareness and prevention services.
• Develop and maintain relationships with community organizations, shelters, healthcare facilities, schools/colleges and other partners.
• Identify opportunities for testing events and outreach in high-need communities.
• Assist with planning health fairs, mobile testing events, educational presentations, and targeted outreach initiatives.
