Job summary

Center for Family and Child Enrichment is seeking an Outreach Worker for the SHAPP/HIV Program in the Miami area. This role focuses on HIV prevention, health education, and linkage to care, serving individuals, families, and communities through field outreach and community events.

  • HIV/AIDS 500/501 Certification (Counseling, Testing, and Linkage) preferred or willingness to obtain within designated timeframe.

Responsibilities

  • Conduct HIV/STI prevention outreach, education, and linkage services to individuals, families, and communities.
  • Provide sexual health education, risk-reduction counseling, and prevention resources related to HIV and other STIs.
  • Promote HIV testing, PrEP/PEP awareness, and access to prevention and treatment services.
  • Engage priority populations including individuals at higher risk for HIV infection, LGBTQ+ communities, and underserved populations.
  • Assist with HIV testing referrals and linkage to care services, ensuring follow-up support as needed.
  • Facilitate small group education sessions and community presentations on sexual health topics.
  • Provide motivational support and health behavior change education to encourage engagement in care.
  • Coordinate with internal CFCE staff and external partners to ensure continuity of care.
  • Support data collection, tracking, and reporting of outreach activities, referrals, and program outcomes in accordance with funding and agency requirements.
  • Enter accurate client and outreach data into Electronic Health Records (EHR) and program databases.
  • Maintain confidentiality and adhere to HIPAA and CFCE privacy standards.
  • Participate in team meetings, trainings, and supervision sessions.

The Center for Family and Child Enrichment (CFCE) is dedicated to help children and their families by providing the right services and solutions based on individual needs. CFCE is constantly evolving to better support our community.

Under general supervision, the Outreach Worker for the Sexual Health Awareness and Prevention Program/HIV Program serves as a frontline public health staff member responsible for community engagement, education, and linkage to care. This position plays a critical role in reducing HIV and STI transmission, increasing awareness of sexual health resources, and improving access to prevention, testing, and treatment services.

The Outreach Worker serves as a trusted liaison between CFCE, community members, and partner agencies. This role focuses on outreach, health education, risk reduction counseling, navigation of services, and supportive engagement with individuals at risk for or affected by HIV and other sexually transmitted infections (STIs). The Outreach Worker supports individuals and communities in increasing health knowledge, reducing stigma, and promoting self-efficacy in managing sexual health through both individual and group-level interventions.

With experience, the Outreach Worker is expected to operate with increasing independence and demonstrate strong community engagement leadership.

Some of the Functions Include:

  • Conduct HIV/STI prevention outreach, education, and linkage services to individuals, families, and communities through field outreach, community events, partner agencies, and CFCE-based programming
  • Provide sexual health education, risk-reduction counseling, and prevention resources related to HIV and other STIs
  • Promote HIV testing, PrEP/PEP awareness, and access to prevention and treatment services

  • Support data collection, tracking, and reporting of outreach activities, referrals, and program outcomes in accordance with funding and agency requirements (e.g., Health Department, CDC, HRSA)
  • Enter accurate client and outreach data into Electronic Health Records (EHR) and program databases
  • Maintain confidentiality and adhere to HIPAA and CFCE privacy standards
  • Participate in team meetings, trainings, and supervision sessions
  • Perform other related duties as assigned
  • High School Diploma or GED required
  • Associate degree preferred in Public Health, Human Services, Social Work, or related field
  • HIV/AIDS 500/501 Certification (Counseling, Testing, and Linkage) preferred or willingness to obtain within designated timeframe
  • Minimum of one (1) year of experience in outreach, case management, or supportive services for at-risk populations, preferably individuals affected by or at high risk for HIV/STIs
  • Experience working with underserved communities, including LGBTQ+ populations, is strongly preferred
  • Knowledge of community health disparities and social determinants of health.

Skills Needed

  • Knowledge of HIV/STI prevention strategies and sexual health education principles
  • Understanding of local community resources, healthcare systems, and social service networks
  • Ability to engage individuals in behavior change conversations and motivational support
  • Strong communication skills, both verbal and written
  • Ability to conduct community presentations and small group facilitation
  • Proficiency in basic computer systems, data entry, and Electronic Health Records (EHR)
  • Strong organizational and time management skills
  • Ability to work independently and as part of a multidisciplinary team
