Job summary

New North Citizens’ Council (NNCC) in Springfield, MA seeks a Temporary HIV/Aids, Health & Prevention Program Outreach Worker to perform community outreach, education, testing, counseling, and resource navigation. The role reports to the Program Supervisor and supports health outcomes in affected communities.

Responsibilities include engaging diverse populations, coordinating referrals, maintaining confidential records, and contributing to program goals. Salary is $25/hour with full-time status.

Qualifications

• High school diploma, GED/HiSET, or equivalent.
• Knowledge of HIV/AIDS, hepatitis C, STIs, substance use, and related community health issues.
• Ability to engage diverse populations with culturally responsive, respectful approaches.
• Strong interpersonal, communication, organizational, and documentation skills.

Responsibilities

• Conduct community-based outreach to promote prevention and health services.
• Engage underserved populations to increase access to education, testing, and support services.
• Provide prevention education, counseling, testing, and linkage to care following protocols.
• Coordinate referrals and follow-up with healthcare providers and partner organizations.
