Job summary

Tulsa CARES Health in Tulsa, OK is hiring for an HIV Prevention & Care Coordinator. The role provides HIV/STI testing, care coordination, linkage-to-care, and client support services for individuals receiving PrEP, PEP, HIV, HCV, and STI-related services.

The position requires a bachelor’s degree in social work with preferred MSW, valid driver’s license, and ability to work with diverse populations. It involves daily field and clinic activities, documentation, and adherence to public health

Qualifications

• Bachelor’s degree in social work (BSW) from an accredited college or university required; master’s degree in social work (MSW) preferred.
• Degree in closely related health, behavioral health, public health, or human services may be considered when education/experience provide necessary knowledge.
• Must maintain a valid driver's license and dependable transportation.
• Must maintain all required licenses, certifications, training, and credentials.
• Experience with barriers to healthcare access and social determinants of health preferred.
• Experience conducting client assessments and coordinating individualized interventions.

Responsibilities

• Provide HIV/HCV/STI testing, including CLIA-waived testing and specimen collection.
• Conduct test result reporting to providers, clients, and required entities.
• Coordinate care linkage and follow-up for clients with positive HIV/STI/HCV results.
• Maintain testing equipment, supplies, and documentation.
• Ensure confidentiality and HIPAA compliance.
• Collaborate with interdisciplinary teams to support client health outcomes.
