Responsibilities

Job Description

The HIV Prevention & Care Coordinator provides specialized HIV/STI prevention, care coordination, linkage-to-care, and client support services for individuals receiving PrEP, PEP, HIV, HCV, and STI-related services. The position applies professional knowledge and independent judgment to assess client needs, identify barriers to care, develop and coordinate appropriate service interventions, facilitate linkage and retention in medical care, and collaborate with medical and interdisciplinary teams to support positive health outcomes.

Primary Responsibilities
  • Provide testing for various sexually transmitted infections including, but not limited to, HIV and HCV.
  • Provides HIV, HCV, STI, and other approved testing services in accordance with agency protocols, public health requirements, and applicable testing standards.
  • Performs CLIA-waived testing and specimen acquisition for clinical laboratory testing within the scope of training, authorization, and applicable requirements.
  • Interprets and communicates testing information within the scope of the position and established protocols and facilitates appropriate referral, follow-up, and linkage to medical care when results indicate additional evaluation or treatment is warranted.
  • Exercises appropriate professional judgment in determining necessary follow-up, client education, referrals, and coordination of services based on testing results and identified client needs.
  • Ensures appropriate specimen identification, labeling, handling, storage, and transportation in accordance with applicable laboratory and public health requirements.
  • Ensure accurate and timely reporting of test results to healthcare providers, clients, and required entities.
  • Maintains testing equipment, supplies, and testing-related documentation in accordance with established standards.
  • Adheres to applicable safety, infection control, quality assurance, and public health standards related to testing services.
Care Coordination/Linkage to Care
  • Provide ongoing follow-up and care coordination for clients identified through HIV, HCV, STI, PrEP, and PEP testing services.
  • Assesses barriers to PrEP and PEP initiation, adherence, and continuation and coordinates appropriate interventions to support treatment access, engagement, and retention.
  • Provide linkage-to-care and follow-up services for clients with a positive HIV test in accordance with OSDH reporting and linkage-to-care requirements, ensuring appropriate documentation and timely follow-up.
  • Coordinate treatment and follow-up for clients with positive STI test results, including communication with medical providers and assistance with connecting clients to appropriate treatment.
  • Provides linkage and care coordination for individuals with positive HCV screening results, including coordination of referrals and follow-up through the agency's medical clinic.
  • Maintain communication with clients and medical clinic staff to support successful linkage, treatment initiation, and follow-up.
  • Document all care coordination, referrals, follow-up attempts, and outcomes in accordance with agency, program, and OSDH requirements.
  • Maintain appropriate confidentiality and comply with HIPAA and all applicable agency, program, and public health requirements.
Other
  • Provides medical case management support as needed and identified by medical provider and supervisor.
  • Participates in formal supervision, professional development, training, and continuing education activities necessary to maintain competency and current knowledge within the position's area of responsibility.
  • Maintains all required licenses, certifications, training, and credentials applicable to assigned duties.
  • Works as part of an interdisciplinary case-management, nutrition, housing, and mental health team.
  • Promptly documents all meaningful client interaction in a thorough and accurate fashion.
  • Supports an environment of health, wellness, dignity, and respect consistent with Tulsa CARES' mission and philosophy of care.
  • Complies with agency policies, procedures, professional standards, and applicable laws and regulations.
  • Complies with Oklahoma State Department of Health standards, including applicable standards governing Counseling, Testing, Referral, and Linkage to Care (CTR-LTC) services.
  • Performs other related professional duties as assigned.
Education & Experience
  • Bachelor’s degree in social work (BSW) from an accredited college or university required; master’s degree in social work (MSW) preferred.
  • Degree in closely related health, behavioral health, public health, or human services discipline may be considered when education and professional experience provide the specialized knowledge necessary to perform the essential functions of the position.
  • Must maintain a valid driver's license and dependable transportation sufficient to perform required job duties.
  • Must maintain all required licenses, certifications, training, and credentials applicable to assigned responsibilities.
  • Experience working with individuals experiencing barriers to healthcare access, including individuals affected by poverty, HIV/AIDS, HCV, chronic health conditions, mental health concerns, substance use, housing instability, or other social determinants of health preferred.
  • Experience conducting client assessments, identifying barriers to care, coordinating individualized interventions, and collaborating with interdisciplinary healthcare or social-service teams preferred.
Knowledge, Skills, & Abilities
  • Knowledge of database software, Internet software, Excel, and Word.
  • Able to collaboratively work with a variety of health and/or social service organizations.
  • Able to maintain profession records through documentation.
