Job summary

Community Health Center is seeking a Prevention Health Navigator for the Center for Key Populations (CKP) to advance HIV prevention through PrEP education, access, and care linkage. The role focuses on reducing barriers, delivering navigation support, and empowering clients to manage their health over time.

Responsibilities include outreach, eligibility assessments, and collaboration with CKP staff to ensure compliant, confidential care coordination within the organization’s guidelines.

Qualifications

• Associate degree in healthcare, public health, social work or related field is required.
• Proficiency in both Spanish and English is required.
• Experience in healthcare settings or community outreach is preferred.

Responsibilities

• Increase awareness of PrEP and provide direct linkage to care.
• Provide navigation services for health insurance enrollment and PrEP provider identification.
• Conduct readiness screenings for PrEP and assess benefits and insurance coverage.
• Assist clients in navigating medical and social systems to manage ongoing PrEP care.
