Job summary

Cook County Health & Hospitals System is seeking an Integrated Care Specialist to conduct HIV testing, outreach, recruitment, and linkage to care, enhancing care coordination within ACHN clinics.

The role supports PrEP education and adherence, with a focus on newly diagnosed or at-risk individuals, under supervision and in a grant-funded setting. Strong collaboration with a team of peer educators is essential.

Qualifications

• High School diploma or GED with 1 year HIV-related health services experience.
• Cultural competence with diverse populations is required.
• Ability to work non-traditional hours including nights or weekends is required.
• Must obtain CDPH/IDPH HIV Testing & Counseling certification within 3 months of starting.
• Proficiency using Microsoft Office is required.

Responsibilities

• Conducts outreach, HIV testing, counseling, and linkage to care at ACHN locations.
• Performs case reporting and health outcomes tracking as required.
• Maintains accurate EMR documentation in CAREWare and other systems.
• Assists in implementing PrEP services and navigation within primary care.
• Participates in weekly case conferences with the care coordination team.
