Responsibilities

• Collaborates with health care team to promote interdisciplinary treatment planning.
• Conducts intake interviews and psychosocial assessments to identify risk factors and arrange referrals.
• Assesses eligibility for health insurance and patient assistance programs; refers and follows up with services.
• Links patients to medical, behavioral health, housing, financial, and support services.
• Monitors progress using EMR and documents adherence to treatment plans.
• Provides therapeutic interventions and educates patients to support adherence to medications and appointments.
• Performs prior authorizations and coordinates with staff for documentation.
• Maintains knowledge of grants/regulatory requirements and ensures compliant patient counseling and documentation.
• Builds community resource networks and stays updated on available services.
• Assists with Outreach HIV/HCV rapid testing and counseling (certification required).
