The role emphasizes adherence support, eligibility assistance for medication access programs, documentation in EMR, and ensuring HIPAA compliance.

Qualifications

• Social service or nursing degree required; MSW preferred.
• Experience coordinating and developing supportive services.
• Experience with HIV patients is preferred.
• Written and verbal fluency in Spanish is highly preferred.

Responsibilities

• Learn and stay current on HIV/AIDS and case management practices.
• Provide case management to clinic patients per HRSA/Ryan White guidelines.
• Create ISP for a ~70 patient caseload and monitor it.
• Coordinate referrals for medical and non-medical services.
• Document patient information in EMR accurately.
