Job summary

DAP Health in Oceanside, CA is seeking an EIS Specialist to identify individuals living with HIV/HCV and link them to timely medical care within our clinics and community referrals.

Responsibilities include delivering HIV/HCV education, assessing barriers to care, coordinating referrals to Prevention Navigators for PrEP/PEP, and maintaining accurate documentation in EHR systems. On-site role with travel across Southern California.

Qualifications

• Bachelor's degree or experience in medical/health fields (social work, counseling, nursing, public health)
• Current BLS certification.

Responsibilities

• Deliver HIV & HCV education and counseling to clients in DAP Health clinics.
• Assess social determinants of health and tailor prevention strategies.
• Facilitate access to medical care and referrals to prevention/navigation services.
• Maintain accurate documentation in EHRs (HCC, Epic).
• Meet clients walk-in and provide post-appointment support.
