Job summary

DAP Health seeks an Early Intervention Services (EIS) Specialist to identify individuals living with HIV/HCV and link them to medical care swiftly through testing, referrals, and outreach services.

The role requires delivering HIV counseling, risk-reduction education, and coordinating with Prevention Navigators for PrEP/PEP; extensive documentation and client navigation are essential. On-site, physician partnerships and health equity focus drive impact.

Qualifications

• Bachelor’s degree or experience in health-related fields.
• Current BLS certification from AHA or Red Cross.

Responsibilities

• Deliver HIV counseling and HCV/STI education to clients.
• Assess social determinants of health to tailor prevention strategies.
• Educate clients on HIV prevention methods and PrEP/PEP referrals.
• Facilitate access to care and remove barriers in the care continuum.
• Maintain accurate documentation and update EHR systems.
• Meet clients walk-in for support and coordinate same-day services.
