Job summary

Santa Fe Recovery Center, Inc. seeks a Disease Intervention Specialist to provide intervention and linkage to care for clients at risk for or living with HIV, Hepatitis C, or related infections.

You will coordinate case management, education, and referrals to testing, treatment, PrEP/PEP, and community supports. Responsibilities include assessing client needs, following up to meet goals, promoting harm reduction, and maintaining strict confidentiality per laws and policies.

Responsibilities

• Provide disease intervention and linkage to care for clients at risk for or living with HIV, Hepatitis C, or related STIs.
• Provide case management including HIV prevention education, care coordination, and referrals to testing, treatment, PrEP/PEP, and supports.
• Assess clients at program entrance and help establish prevention and treatment goals for sexually transmitted infections.
• Follow up to ensure basic needs are addressed and documentation is completed to enable progress.
• Liaise with community services and other case managers to assist clients with goals and recovery needs.
• Educate clients on harm reduction, promote healthy resources, and risk-reduction training.
• Uphold client confidentiality per policies and laws; maintain case files with residential/outpatient teams.
• Ensure GPRA documentation is completed in a timely manner.
• Collaborate with MAI Coordinator and grants for data tracking and reporting.
• May attend community events to promote health information and linkage to resources for prevention, screening, and treatment of STIs.
• Other duties as assigned.
