Job summary

• Locate patients who have fallen out of care using available records and outreach strategies.
• Conduct home, hospital, and community-based visits to support continuity of care.
• Document client interactions and progress in CAREWare per HIPAA guidelines.
• Collaborate with outpatient case managers to remove barriers to care and ensure patient satisfaction.
