Job summary

Prisma Community Care Phoenix is seeking a Ryan White Case Manager to provide comprehensive case management for clients living with HIV in the Phoenix area. You will assess needs, develop service plans, and coordinate care across medical and social services to promote self-management and viral suppression.

You will work with internal teams to ensure timely documentation, insurance navigation, and access to housing, food, and supportive services, with an emphasis on culturally competent care for

Qualifications

• Bachelor’s degree in social work or related field, or 4+ years of case management experience with a high school diploma or equivalent.
• Ability to communicate effectively with diverse populations affected by HIV and social barriers.
• Knowledge of HIV, housing, insurance, and community resources to support clients.
• Understanding of gender identity and sensitivity within clinical and community settings.
• Proficiency in Microsoft Office and EMR systems with HIPAA compliance and documentation skills.

Responsibilities

• Conduct psychosocial assessments and eligibility screenings for Ryan White Program clients.
• Develop individualized service plans to address medical, behavioral health, housing, and social needs.
• Assist clients in navigating care to improve attendance and treatment adherence.
• Manage a caseload with productive time spent on billable Ryan White activities.
• Record contacts and referrals in EMR within required timelines.
