Job summary

PrimeCare Health is seeking a Medical Case Manager - Ryan White in Chicago, IL, to provide patient-centered, confidential services to individuals living with HIV/AIDS and facilitate access to core medical and support services. The role emphasizes helping patients achieve an undetectable viral load by removing barriers to care.

You will manage a caseload, conduct intake assessments, develop care plans, maintain records, collaborate with healthcare teams, and participate in outreach as needed.

Qualifications

• Demonstrated knowledge and at least six years of work experience equivalent to one of the stated education credentials.
• Ability to work with diverse patients living with HIV/AIDS and coordinate care services.
• Experience with evaluating applications and assessing patient needs and barriers to care.

Responsibilities

• Oversee and manage an assigned caseload of patients living with HIV/AIDS, providing comprehensive Medical Case Management services.
• Complete intake into care, including comprehensive assessments of patient strengths and needs.
• Maintain knowledge of resources and develop referral databases to connect patients to services.
• Develop comprehensive care plans and monitor progress toward goals of care.
• Maintain patient files per PrimeCare Community Health and AFC standards.
• Collaborate with medical teams, hospitals, and insurance entities to provide integrated care.
• Participate in outreach, trainings, and development projects as needed.
