Job summary

Codman Square Health Center Inc in Boston seeks a Medical Case Manager to provide medical and social services case management for adults with HIV/AIDS, advocating for patients across clinical and social determinants of health. You will coordinate referrals for medication assistance and navigate health services in collaboration with a multidisciplinary team.

The role emphasizes adherence support, patient engagement, and up-to-date documentation in EMR/EHR systems.

Qualifications

Associates degree or minimum of 2-years progressive professional experience.
Knowledge and experience with HIV services, including education/prevention, outreach, counseling and care management.
Awareness of substance abuse issues, including detox, rehabilitation and recovery.
Ability to work within a wide community and social service network.
Proven ability to work with patients from diverse backgrounds.
Familiarity with EMR/EHR systems and documentation skills.
Ability to speak Haitian Creole highly desirable.

Responsibilities

Provides case management services to HIV positive patients.
Supports new patients with intake and onboarding, acclimating them to the clinical processes and providing emotional support.
Reinforces self-determination and utilizes patient teaching opportunities to ensure medication adherence and engagement
Collaboratively works with MD, RN and other MCMs to ensure optimal care
Refers clients to community agencies to support SDoH needs
Conducts monthly office/telephonic visits, updating plans of care and supporting outcomes
Maintains monthly activity and variance reports on high-risk patients
Attends team meetings as requested by clinical leaders
Completes paperwork within required time frames
Performs other duties as requested
