Job summary

Codman Square Health Center Inc. is seeking an HIV Nurse Case Manager to provide client-centered clinical case management for adolescents and adults living with HIV/AIDS.

You will coordinate access to primary care, medications, and supportive services in collaboration with the HIV care team. The role requires an MA RN license, CPR certification, and 3+ years nursing experience with emphasis on case management and HIV knowledge; bilingual skills are a plus.

Qualifications

• BSN or equivalent nursing degree; MA RN license required.
• Massachusetts RN license and CPR certification.
• 3+ years nursing experience; 2+ years in acute care/medical-surgical settings.
• 5 years clinical experience preferred.
• Previous case management experience preferred.
• HIV/AIDS knowledge or experience preferred.
• Strong health promotion and disease prevention knowledge.
• Community outreach experience preferred.
• Excellent interpersonal and communication skills.
• Strong computer skills.
• Self-motivated and able to work independently with minimal supervision.
• Bilingual in Spanish and/or Haitian Creole desired.

Responsibilities

• Provide client-centered clinical case management to adolescents and adults living with HIV/AIDS.
• Develop, monitor, and reassess individualized service plans (ISP) within 60 days; review every six months.
• Coordinate access to primary care, medications, treatment adherence, and specialty care.
• Educate patients and families; support self-management and harm reduction.
• Collaborate with clinical team; prepare reports and present updates.
• Conduct home and hospital visits when needed; ensure follow-up and attendance.
• Maintain documentation and complete required paperwork timely.
• Participate in trainings and professional development.
